THE OPPORTUNITY

My Health Coach is a patient centered platform of the Health Services Department in Physician Partners. This innovative team member will work with the team to accomplish our overall mission of “Better Health.” As a team we provide the highest quality education, resources, and risk assessment in healthcare today. We work to inform our members on preventative measures and engage them in their own health and well-being. You will collaborate with the health services team to devise solutions for obstacles to care within the markets we serve. The role operates in a fast paced, but highly flexible and engaging environment.

THE WORK

A star Health Coach will drive the My Health Coach platform to the reduction in utilization of acute care services and promote positive patient outcomes. They will encourage the use of patient centered medical home tactics within physician offices, and collaborate physicians and telephonic health coaches to implement innovative care plans for high cost high needs patients. The Health Coach will not only be systematic, but also adapt flexibly to changing priorities as our company identifies new opportunities.

In this role you will collaborate and partner with physicians, their office teams, and other members of the Physician Partners team to:

  •  Foster physician engagement with the Care Coordination platform
  •  Promote patients timely access to appropriate care
  •  Increase utilization of preventative care
  •  Reduce emergency room utilization and hospital readmissions

You will leverage direct communication with physicians, a suite of company tools, and our in-house technology platform to accomplish these goals.

What Success Will Look Like:

  • Reduction in patient utilization of acute care services
  • Changed PCP mindset from fee for service to patient centered medical home
  • Established robust relationships with affiliated physicians
  • Development and implementation of innovative health delivery initiatives within your region
  • Medical Director  involved only at most strategic level of regional activities
  • Improved health outcomes for our members through excellently managed delivery of healthcare

KEY RESPONSIBILITIES

  • Track admits region wide
  • Identify D/C and pts PCP
  • Schedule initial f/u apt within 48-72 hours with PCP
  • Communicate with assigned HC in regards to transition of care (TOC)
  • Monitor LOS
  • Identify LOS >10 Days
  • Communicate with assigned HC to initiate complex case management
  • Assist with TOC of all members
  • Hospitalizations
  • SNF
  • Support Request
  • Complete support request submitted by the networking team as they complete their monthly meetings with the providers.
  • Identify those complex requests that will need additional assistance
  • Communicate those individual cases to the HC assigned to the particular PCP
  • Respect patients and maintain confidentiality per HIPAA regulations
  • Coach physicians on hospital admission management
  • Innovate strategies with physicians to care for High-Cost High Need patients
  • Engage office staff in patient-centered medical home tactics
  • Function as a part of a larger care team
  • Serve as the physician's main point of contact for needs related to care coordination

EDUCATION AND EXPERIENCE

  • Customer Service experience is a must
  • Previous call center experience preferred
  • Previous MSO or Health Plan knowledge a plus
  • Experience working with geriatric populations
  • Strong problem solving skills with great attention to detail
  • Excellent ability to organize and follow-up
