Care Coordinator - Ocala - Medical Assistant or LPN

The  Care Coordinator is an embedded position that works directly within an affiliate physician office to guide them on the delivery of high quality preventative healthcare to our members.  You will collaborate with physicians and office staff to devise innovative solutions to improve the quality of healthcare delivery.  You will leverage direct communication with patients and physicians, possess an incredibly caring attitude and utilize our in-house technology platform to accomplish these goals.


In this role you will collaborate and partner with physicians, their office teams and other members of the Physician Partners team to promote timely access to appropriate care, increase utilization of preventative care and reduce emergency room utilization and hospital readmissions.  The successful candidate will increase care plan comprehension through culturally and linguistically appropriate education, and increase continuity of care by managing relationships with tertiary care providers and referrals and connect patients to community resources.

  • Three years or more of clinical experience, as a Medical Assistant or LPN
  • Experience working with geriatric populations
  • Exceptional interpersonal skills and caring demeanor
  • Strong problem solving skills with great attention to detail
  • Excellent ability to organize and follow-up
