Regional Lead Care Coordination

The Regional Lead of Care Coordination reports directly to the Medical Director of a successful, high-growth healthcare company. This role works with our affiliated physicians to guide them on the delivery of high quality preventative healthcare to our members. You will collaborate with the regional managers from other departments to devise solutions to meet our rapidly changing business needs within your markets. The role operates in a fast paced, but highly flexible and engaging environment.


In this role you will collaborate and partner with physicians, their office teams, and other members of the Physician Partners team to:

  •  Foster physician engagement with the Health Coach platform
  •  Promote patients timely access to appropriate care
  •  Increase utilization of preventative care
  •  Reduce emergency room utilization and hospital readmissions

You will leverage direct communication with physicians, a suite of company tools, and our in-house technology platform to accomplish these goals.

  • Engage office staff in patient centered medical home tactics

  • Exceptional interpersonal skills and caring demeanor

  • Active RN license in good standing and without restrictions in the State of Florida
  • Three years or more of clinical experience in a managed care environment
  • Previous MSO or Health Plan experience
