THE OPPORTUNITY

The Telephonic Care Coordinator is office based and is not a remote position.  In this role you will ensure the delivery of the high quality preventative and proactive health care in an effort to reduce emergency room and hospital utilization. You will also work closely with physician office staff on the implementation and utilization of Patient Centered Medical Home tactics. 

THE WORK

The Telephonic Care Coordinator will advise primary care physicians about practice improvement opportunities. A successful candidate will encourage physicians to implement best practices in medical management, and help practices implement a Patient Centered Medical Home model of care.

KEY RESPONSIBILITIES

  • Identify and close gaps of care for High-Cost High-Needs patients
  • Promote patients timely access to appropriate care
  • Increase utilization of preventative care
  • Reduce emergency room utilization and hospital readmissions
  • Coordinate and administer clinical programs with primary care physician offices
  • Proactively identify new and existing patients with complex conditions that would benefit from care coordination activities

EDUCATION AND EXPERIENCE

  • LPN Florida License Required
  • Geriatric primary care experience preferred
  • Case management, case coordination or medical administration experience is preferred
