Care Coordinator - Tampa - Medical Assistant or LPN

The Care Coordinator is a critically important part of our team. The role manages our most important physician relationships. This role works with our affiliated physicians to guide them on the delivery of high quality preventative healthcare to our members. You will collaborate with the regional managers from other departments to devise solutions to meet our rapidly changing business needs within your markets. The role operates in a fast paced, but highly flexible and engaging environment.


In this role you will collaborate and partner with physicians, their office teams, and other members of the Physician Partners team to:

  •  Foster physician engagement with the Care Coordination platform
  •  Promote patients timely access to appropriate care
  •  Increase utilization of preventative care
  •  Reduce emergency room utilization and hospital readmissions

You will leverage direct communication with physicians, a suite of company tools, and our in-house technology platform to accomplish these goals.


  • Coach physicians on hospital admission management
  • Innovate strategies with physicians to care for High-Cost High Need patients
  • Engage office staff in patient-centered medical home tactics
  • Function as a part of a larger care team
  • Serve as the physician's main point of contact for needs related to care coordination

  • Three years or more of clinical experience as a Medical Assistant or LPN
  • Experience working with geriatric populations
  • Exceptional interpersonal skills and caring demeanor
  • Strong problem solving skills with great attention to detail
  • Excellent ability to organize and follow-up
